How to receive the complimentary 10 Trip Book of Tickets
To qualify for the 10 Trip Book of Tickets, individuals must have registered and received
the complimentary interview rides. Plus, those who have registered must also provide the
MMVTA type written proof that they were hired by the employer who they originally listed
during program enrollment. MMVTA will accept proof of employment via the following:

• By filling out the form below and attaching your proof of employment (MMVTA
  will accept scans of your proof via PDF, TIFF, JPG or Microsoft Word formats)

• Fax: 724-489-0750

• Email attachment: customersupport@mmvta.com

• US Mail: MMVTA, 1300 McKean Ave., Charleroi, PA 15022

• Or: In person at the Transit Center located at address above.

NOTE: DRIVERS will NOT Books of Tickets for this program. All complimentary 10 Trip
Book of Tickets will be sent to qualifying individuals via US Mail to the address that was
given to the MMVTA during initial enrollment.
